Off-the-Shelf CAR T Therapy Cema-Cel Shows Strong Early Results in Lymphoma Trial

Interim results from the phase 2 ALPHA3 trial suggest that cema-cel, an off-the-shelf CAR T-cell therapy, may significantly improve outcomes for patients with large B-cell lymphoma who remain MRD-positive after initial treatment. In this high-risk group, the therapy achieved MRD negativity in 58.3% of patients, compared to just 16.7% with standard observation. Circulating tumor DNA levels also dropped sharply, with a median clearance of 97.7% by day 45, versus 26.6% in the observation group, highlighting a strong early anti-cancer effect.

Cema-cel demonstrated a favorable safety profile, with no reported cases of cytokine release syndrome, neurotoxicity, or graft-versus-host disease—side effects commonly associated with CAR T therapies. Because it is an allogeneic, ready-made product, it eliminates the need for individualized cell manufacturing, which can delay treatment. This approach could allow for faster delivery and expand access, potentially enabling use in outpatient settings.

The trial is enrolling approximately 220 patients with large B-cell lymphoma who remain MRD-positive after first-line therapy. Participants receive either cema-cel following lymphodepletion or standard observation. The study’s primary endpoint is event-free survival, which remains blinded.
