The Phase 2 BRCAAway trial shows that combining abiraterone, prednisone, and olaparib is far more effective than using either drug alone for patients with metastatic castration-resistant prostate cancer who have BRCA1/2 or ATM mutations.
Patients receiving the three-drug combination lived a median of 68 months—over five years—compared with 28 months for abiraterone alone and 37 months for olaparib alone. Progression-free survival was 39 months with the triplet versus 8.6 months for abiraterone. PSA levels dropped significantly in 95% of patients, with 38% reaching undetectable levels.
Using all three drugs together from the start proved much better than giving them one after another. Despite the stronger treatment, the combination was well tolerated, and many patients stayed on therapy for years with very few severe side effects, making it a promising long-term option for aggressive prostate cancer.
