A new combination therapy for advanced urothelial cancer shows promising results, according to the Phase 2 TROPION-PanTumor03 trial. Datopotamab deruxtecan (Dato-DXd), a TROP2-targeted antibody-drug conjugate, was combined with rilvegostomig, a bispecific antibody against PD-1 and TIGIT, to enhance antitumor activity.
In cisplatin-ineligible, first-line patients (n=22), the confirmed objective response rate (ORR) was 68.2%, with a 12-week disease control rate (DCR) of 95.5%. Median progression-free survival (PFS) was not yet reached, suggesting durable benefit. In platinum-pretreated, second-line patients (n=18), the ORR was 38.9%, DCR 83.3%, and median PFS 12.5 months, demonstrating meaningful activity even after prior chemotherapy.
The safety profile was consistent with previous studies, with common side effects including oral mucositis, nausea, and ocular events. Dose reductions were required in some patients due to treatment-related adverse events. These results support further investigation of Dato-DXd plus rilvegostomig as a novel first-line option for advanced urothelial cancer.
